How much information do you need to get my records?
When you add a new doctor or facility, you’ll be asked the doctor’s name and specialty, the facility name, the location of the office/hospital, and when you were last seen there. The more information you can provide, the easier it will be to find your records! That being said, we’ve gotten pretty good at tracking down records based on just the doctor’s name and location. If we have trouble finding your records for any reason, we’ll reach back out to you for more information.
