Why do you need my Social Security Number?

We need the last 4 digits of your social security number (SSN) in order to retrieve your medical records from your doctor. Most doctor’s offices require your date of birth and the last 4 digits of your SSN to confirm your identity and confirm that you have authorized us to get these records. If you participate in research, information that can identify you personally (including the last 4 digits of your SSN) is never shared with researchers.
